FINAL REMINDER: Annual information forms must be completed by midnight tonight in the PowerSchool Parent Portal:

Grade 2, 5 and 8 families are the ONLY ones who will see the Annual School Change Residency Verification. That is the ONLY residency need to be completed by those grades.  ALL other families should complete the Annual Residency Affirmation statement.

ALL families should complete the Student Information, Health Information, and Transportation Request forms IN ADDITION to the residency requirement.

When trying to upload a physical or an immunization record on the health form:
1. Click the sign in button
2. Re-enter you PowerSchool credentials
3. Click Register
4. Agree to the terms of service
5. Upload
